Hi Drew,
yes, I found the form to be clear and simple (I mean that in a good way).
I only wish I had started off with something similar. As it is, I just have tons of paper in files.
I have just started to sort them out, as I try to make up my booklets.
Having completed three, I have a loooooong way to go.

I'll be at the library sometime in August. Ma-in-law lives in Bonhill.
